Helplessness can be as simple as state of mind.
True, for some, the concept requires an unyielding capture but for others the command of another can be enough. A sub taken to her/his knees by their Dom can be enough to trigger a feeling of both submission and helplessness. It really depends how deeply you believe in your submission. The addition of restraint, no matter how slight, only adds to those feelings.
