(Without any spoilers from blood and wine, since I've not played that yet)
Four or so years after the events of Witcher 3, Ciri (now a full blown Witcher) arrives on the shores of Skellige and is attacked. Due to exhaustion from her travels she is unable to fight them off and is nearly beaten until Cerys (now queen of Skellige) helps her survive before getting her back to health.... one way or another
And then we'll carry on however we see fit.
