TLDR ABO setting with some changes including but not limited to: females still being present and, depending on genre, looser gender dynamics.

This is all more of an outline rather than a strict settings so if there are changes you don't like or things you want to alter a little then by all means go ahead and share them. Only real rules are no rl face claims and follow site rules. As far as themes and kinks go, I'm really open so don't be shy about sharing anything weird so long as it doesn't involve filth or gore.

Females are still present, though they are all infertile unless they chose to become an Alpha and cannot become Omegas.
At birth everyone is a Beta, and upon reach adulthood (18) they are given the choice to become an Alpha, Omega, or remain a Beta.
The change to an Alpha or Omega comes with more than just the ability to reproduce, for some its as little as a change in height and for others its as extreme as gaining entirely new features.
Typically I have the gender dynamics be less restricted, but I understand that is the appeal for some so its more on a story by story basis rather than a static change.

What many tales of heroics fail to mention is the repercussions of interrupting a nearly complete sorcerous working. Usually there is a big flash of light and nothing seems to change, the villain dies and the world carries on as it was. Well in our story there was more than a flash of light. The magic caused a change in the natural order. Years down the line, long after the heroes have died or ascended to godhood, the magic finally took effect. Women were no longer able to conceive, regardless of species they could not bear child.
Druids, Wizards, Clerics, Occultists, none could find a way to reverse the change. It came down to the often looked down on Alchemists to save the sentient races. Rather than seeking to undo the change, they instead made a change of their own. With a mix of their chaotic science, highly toxic substances and a pinch of magic, they produced an elixir that would allow people to reproduce again. Unfortunately there was no way of telling which role you would take, and the side effects were quite extensive. Still, it was either mutate or go extinct and many people chose to change.

Its been centuries since the last recorded female pregnancy. Since that time it has become a requirement that upon reaching adulthood each person makes an appointment with a specialist to discuss their future. The options are all rather straight forward, though the impacts are quite heavy.
Those that opt to remain a Beta end their meeting there, are given a card with a number to call if they change their mind and are sent on their way to return to their life. Certain beliefs and cultures frown on not making a decision, but there is no law requiring someone to choose.
Those that opt to become an Alpha are signed up for classes to ensure they know what they are signing up for, and after completing the classes they undergo a series of treatments. For most its as simple as taking a pill and dealing with a month or so of growth and spiked libido. For some its a longer ordeal that can take up to a year before they are considered finished with treatment, though this is rare.
Omegas have a slightly rougher time of it. They have to undergo medical exams to ensure they can safely undergo the treatments, to say nothing of the intense treatments they undergo in order to be able to bear a child. Even after that they often have frequent check ups to ensure things remain safe for them.
While Alphas and Omegas are legally no different than Betas, there are certain expectations and biases that arise from undergoing such extreme changes.
