So, recently I read an interesting story but I unfortunately can't remember exactly where I found it anymore. I think it was on deviantart, but it could have been somewhere else.
In any case, the content inspired me for the scenario that I would like to suggest to you here.
I would like to mention three things in advance, though.

My character's name is John. He is the son of YC (either his mother or both mother and father) and still lives in the house with YC as well, as he is still in his last year of high school.
Overall, he, like the whole family, has led a very average life to date. They belong to the middle class, John gets good but not outstanding grades and he has very modest plans for his life.
But a tragic mistake is about to turn this very ordinary life upside down. A few days ago, John's family doctor discovered that he had a slightly inflamed appendix. Nothing dramatic. The diagnosis came early enough that a surgical procedure could be scheduled a few days later.
The operation was to be carried out that morning and at first everything seemed to be going according to plan. John was said goodbye to by his parents and taken to the operating room, where a friendly and nice assistant administered the general anesthesia. John fell asleep and didn't notice anything about the whole operation.
The hours passed. Such a procedure, which is nowadays really a routine, doesn't actually take very long, which is why his parents slowly started to worry when John didn't come back from the operating room. But each time the nurses calmed them down. They had asked the surgeon in charge and everything was going according to plan.
In fact, the doctor treating him couldn't understand why his patient's relatives seemed so nervous. After all, he did exactly what he was told to do - a gender reassignment operation...
Yes, that's exactly what it said on the form that was given to the doctor before the surgery started and was supposed to provide information about the agreed procedure. However, John's form seemed to have been swapped with that of another patient, so that not only his appendix was removed as planned, but also at the same time everything biologically male as well.
At least the doctor treating him was very careful, so that during the operation he realized that the appendix would definitely have to be removed too. So John at least received the treatment he had actually come for.
John himself was completely unaware that his entire life was about to change forever. You can't really call it luck in a moment like that, but the doctor who operated on him was one of the best in the field of gender reassignment surgery. He formed a vagina out of John's penis that was almost indistinguishable from a natural one, inserted a hormone implant that released estrogen into his body fully automatically, and feminized John's facial features and even his voice. The doctor was most proud of the fact that he was the only doctor to date to be able to implant an artificial but fully functional uterus.
The patient with whom John was swapped had opted for the complete package, so after the operation John was not taken to the normal recovery room, but to a special infusion room. While still asleep, he was given a high-dose estrogen infusion for a few more hours, which put the finishing touches to his new feminine appearance. So he grew a natural and quite ample bust and a decent butt. In general, his body fat increased in the right places, concealing his masculine, distinctive features. His hair also experienced a growth spurt on his head, while it largely fell out on the rest of his body.
After what felt like an eternity, John finally woke up - in a completely different body.

How does John deal with this drastic change, but also how do his parents or his mother (depending on how many characters you want to play) deal with all this? I'm not just hoping for smut, but also a lot of plot and character development in this scenario.
At the same time, smut can of course be an important part of this scenario as well. I could imagine that John's mother would always have liked to have a daughter, so that after overcoming the initial shock, she even enjoys teaching John what it means to be a girl. Going shopping, putting on make-up and yes, helping him discover his new sexuality can all be part of the role play.
As I said, this can become a lesbian scenario, but also one in which a father is involved.
What's most important to me is a wholesome atmosphere
