In the book, Lord General Tobias Brogan captures Duchess Lumholtz using magic and slices off her left nipple. The nipple is then used as a medium to enslave the duchess. Her left tit is healed but is now missing a nipple.
Tobias then hands her over to his aide Galtero to rape. Tobias offers to order the duchess to have sex with Galtero, but Galtero refuses and wants the duchess to struggle.
He then drags off a screaming duchess to rape.
