Help me with a survey for class

So for my English 220 class, we needed to make a survey (check) and then collect data from people. The collection will go on for about a month and a half, and I'd appreciate it if some people did my survey. The topic is: How the COVID-19 Pandemic Has Affected Interpretation of Meaning in Conversation. I'd really appreciate any people's answers

I wanted to help out and fill in said survey, but I must be logged in to Google to take part. And that goes against my principles of taking part anonymously. If you could provide a link where that is possible, I'll happily take part, otherwise, good luck with it.

The only reason you need to sign in is so that it is harder for one person to do it multiple times. It will not inform me of your email or anything about you that isn’t answered in the survey. Which at most is your age range.
