She revealed the pregnancy in her last letter to Sirius before she died. In that scene where Snape had broken into Grimmauld Place (in The Deathly Hallows) after Dumbledore died, he discovers that she was pregnant with a baby girl at the time of her murder. Rummaging through Sirius' possessions, Snape finds more letters about Lilly and James' secret daughter along with a diary revealing the part Sirius played in covering up her existence.
As the first person to arrive at the scene after Lilly and James' death, Sirius' diary reveals how he performed a quick procedure on Lilly to rescue the premature baby she carried in order to save it's life. He placed it temporarily on the door of some muggle hospital where a witch he knew worked undercover. He never got a chance to return to visit the child after his capture, though he wrote regularly to check on her progress from Azkaban. The undercover witch (via Sirius' instruction), arranged for her adoption to a loving muggle couple, and kept a close eye on the family under the guise of a family friend.
From the letters between Sirius and the witch, Snape deduces that the girl has illegally had her powers bound the night she was 'born' so that the magical trace placed on all infants under the age of seventeen would never be activated. Thus, after she turned seventeen, she would fall off the Wizarding radar completely. In turn, it would make it easier for her to fit in and hide in the muggle system safely.

Unable to resist the curiosity, he visits her and is instantly overcome with shock at her striking resemblance to Lilly at sixteen. Even down to her mannerisms with that strange mix of both innocence, sweetness and fierce independence. He's overwhelmed with a desire to possess her and keep her when he has the opportunity. Before she, like her mother, ever gets the chance to wriggle out of his grasp forever.
